Jack E. Smith
Born May 17, 1949, In West Virginia, He left us quietly on August 8th 2011. At the Hospice House of Sarasota.
He is survived by his wife Kathryn Parks-Smith.
His Three Little Birdies: Alisa, Summer, and April.
Four grandchildren: Samantha, Lee, Cameron, and Aerial.
Five brothers and sisters: Elwood, Alethea, Peggy, Buddy, and Linda.
